PART XXX SHAFTS, HOISTS AND CONVEYANCES — Machinery record book

(1) An employer shall ensure that there is recorded in the machinery record book an entry of
628(a)
every examination and test that is required under these regulations, as specified in the book;
628(b)
a failure of or accident involving the hoist, rope, conveyance or shaft; and
628(c)
a correction of or repair to, the hoist, hoist controls, signalling systems, hoisting rope, shaft conveyance or another part of the hoisting, dumping or loading equipment.
628(2)
An entry referred to in subsection (1) shall be signed by the worker making the examination or report.
628(3)
The entries referred to in subsection (1) shall be read and signed by the supervisor of the worker performing the functions referred to in subsection (1).
