PART V COMPENSATION AND RIGHT OF ACTION — Duties of employer

the occurrence and nature of the injury;
65(b)
the time the injury occurred;
65(c)
the name and address of the worker;
65(d)
the place where the injury occurred; and
65(e)
other particulars required by the commission.
65(3)
An employer shall make additional reports respecting the injury and workers that may be required by the commission.

(1) A n employer shall give the commission written notice of any, or the possibility of any, work-related injury to a worker that comes to the attention of the employer within 3 days of receiving the information.

The written notice referred to in subsection (1) shall include the following information:
